SCHEDULING AN APPOINTMENT
There are two convenient ways you can schedule your appointment with our offices, by filling out the online form and then a representative
will contact you and confirm your request or by calling the practice of your choice listed below.
BEFORE YOUR APPOINTMENT and to help reduce the wait time we now have "online registration" which will enable you to fill out your patient
information form electronically. Please click on the ONLINE REGISTRATION link below. You will also need to view the Binding Arbitration video
prior to your appointment. Please click on the "View Video" link below.
Additional forms required for your appointment type are listed below. Please complete the form(s) that best represent your upcoming
appointment and bring them with you to your scheduled appointment. Also, please view the Binding Arbitration video below. When you check
in the front office staff will ask you to sign a form indicating that you have viewed the video.
Please remember to present your most updated insurance card to ensure the information is valid at the time of service. Incorrect or
out-of-date information will delay your claim and may result in full payment by you.
Co-pay and deductibles are collected prior to service rendered.
