• Calling the appropriate office location

Prior to Your Appointment:  

• To make scheduled appointments as smooth as possible and to help reduce wait times, patients should watch the Binding Arbitration Video and complete any necessary documentation located under Patient Forms.  At check-in, patients will be asked for all necessary form(s) and to sign the Binding Arbitration Video Form, indicating the video has been viewed.
• Please present your most up-to-date insurance card to ensure the information is valid at the time of service.  Incorrect or out-of-date information can delay your claim and may result in full payment by you.

Copayments and/or deductibles are collected prior to services being rendered.
